We have recruited Paramedic Practitioners who are clinically trained to see all types of patients. They are currently not independant prescribers. They are supervised by our GPs.

The surgery is closed everyday for lunchtime between 12:30 and 13:30. The phone lines will be open for emergency calls only during this time with normal service resuming at 13:30.

Online 'Pre-Registration' With The Practice

 

If you wish to pre-register click on the link below. This does not guarantee registration. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. Alternatively please print the pdf copy below, complete and bring to the surgery with your ID.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct. You may wish to bring an encounter report from your last surgery and proof of any repeat medications.
